National Healthcare Decisions Day is a day to encourage people to document their healthcare wishes and empower individuals to make informed decisions about their end-of-life plans. Beacon Hill Village will be hosting drop-in hours from 10:00 AM to 3:00 PM for members to stop by and update their forms. We will have copies of the Health Care Proxy form, Personal Directives, Five Wishes, and Files of Life available. Witnesses will be available if you need to update your health care proxy as well as a notary if you need to notarize your durable power of attorney or other documents.
